That, and also this was in the '90s - how long did it take for companies that weren't using the best practices to see enough failures to realize their mistakes and address them? Dollars to donuts the failure rate among the old hands has been steadily dropping since the '80s.

Airplanes are full of welded titanium hydraulic lines. How often do you think those welds fail?
