I've been the exact opposite on my Lake shoes. The 237 are 45, and the 402 are 44.5, based on the 45 fit. Both work, but the 45 size can take extra socks, if need be.
Agree on custom, other than the fact that every custom shoe I've seen looks like a club hanging off that person's ankle.
