The Cinelli 64 is my favourite 'bar on a race bicycle and you could buy 42s and 44s in the 1980s - I did, and still have them. On touring bicycles I use the super-rare Cinelli Campus handlebar, the Nitto (B135 in 45cms) and SR Randonneur (44)
On older bicycles (50s) I use a GB Randonneur handlebar - I have quite wide ones.

back in the day is a relative term, and has different meaning for different people. I was thinking of the crest logo days. I am reasonably certain that only the model 66 came in 42 up until 1985. After that, I'll take your word for it that the mod 64 became available in larger sizes. Perhaps availability was different in the UK.
Interesting that GB bars came in larger sizes.
How do you like your B135 bars? I was thinking of switching to a traditional randonneur bend. My current bars are Nitto B172, which is a modern compact soft ergo bar. I find the drop position comfortable, but it's easier to grab the brake levers from the drops with a traditional bend.
