Both times that I had a broken cable, the symptoms weren't at all obvious on previous rides. So I've never noticed a fraying cable by checking at home.
After getting caught by surprise out on the road the first two times it happened, (once on Campy and once on Shimano), I finally wised up, and caught the third one
before it broke by inspecting it.
