I slacken off the cable entirely before adjusting limit screws; those limits are set solely to suit the wheel/cassette.

If you mean top gear = smallest/outermost cog... before any adjusting, first thing I'd check whenever it ain't reaching high gear is if the RD has taken a bump and has been bent inwards.
