Actually happens more often than we think. I remember seeing pics of a lot of Campy Campy cranksets cracking apart like that and in other areas around the spider. Usually starts out as an invisible crack that works through the material for years and many cycles of stress and release, that is why it will he dark where the crack originated, from corroion and light where it finally cracks apart. I suspect the threads cut into the crankarm at the pedal axle hole is where the crack might have started because of "stress risers".
