I think that this similar to how the Dia-Compe Royal Grand Compe works. Since the threads are only for the adjuster nut and doesn't thread into the arm, maybe just filing off the threads on one side of the barrel would work. The flat side is just to keep the barrel from rotating when you turn the adjuster nut to increase/decrease the cable tension.
