2mm toe-in is standard for most pads. If that doesn't work, use different pads. I had that problem on one of my road bikes after I replaced the rear pads (Shimano Ultegra pads on D-A dual pivot brakes) - I tried various amounts of toe-in, cleaning the rims, sanding the rims, nothing helped. I finally installed a pair of Kool Stop road brake pads I had in my junk box, and that worked.
