Pull out the stem and make sure the bolt is threading into the wedge and not just turning it in circles. Screw the bolt far enough into the wedge so it only need a few turns to tighten it. You can even tighten it outside the steerer tube and then back the bolt off a little to fit it into the steerer.

The other possibility is the threads are stripped. I haven’t experienced that but the environments some bikes go through can cause most anything.
