Every manufactured item can have hidden flaws or a quirk in the structure or a bubble in the casting.
All it takes is an opportunity to show itself.
Should be an easy rebuild.

That was my thought too, possible defect in the steel.
Yes we put in a spindle, cranks (same chain ring size) and a set of pedals from our "salvage" stock along with new bearings and grease. Everything else checked out so It is ready to be given out.
