The only way that can happen is if the cable is slipping. Put a Sharpie mark on the cable where you cinch it down, and see if it's slipping. Taking a little bit of sandpaper can help remove the slick coating on cables at the location where they get bolted down.
This is a major reason I do not care for coated inner cables.
