I'm no lawyer, but I think insurers would be within their rights to deny reimbursement for treatment directly related to COVID to people who have been offered vaccination and refused without medical justification. That's not at all the same as denying care and insurers pick and chose what they'll reimburse all the time.
The problem with this approach is that it’ll be the hospitals and clinicians eating that cost when people simply refuse to pay.
