Learned a bit of an expensive lesson about 50.4bcd chainring bolts that day...I'd heard blue threadlocker recommended for these kind of bolts and now I know why.
Had one fall out of a Campagnolo crankset and mtb cranks, too. I always use blue Loctite on them now.
