That is a nice looking crown. Kind of looks like Cinelli MR and CCA had a baby:






I think the fork was most likely at some point attached to a 1989 Bridgestone (Synergy) RB-1 (or possibly RB-2?). The 'Cr-Mo Tubes' stickers are original (forks painted white had the inverted, black-on-white decal scheme). It is odd that the catalogs for both 1988 and 1989 show the same crowns for RB-1 and RB-2, neither showing the one you have here.

As for the crown itself, I'm not sure who made it but it does look an awful lot like the Takahashi ICR-6 that shows up in one of their catalogs (sorry, this blurry scan is the only one I have):
