I toe out a lot and I wouldn't be able to use clipless pedals without pedal extenders. Even with the extenders my heel rubs the stay and the crank on the right side. Some bikes and some shoes are worse than others. My Cannondale CAAD5 had a polished spot near the derailleur but it didn't wear through. The shoes I used for years have a hole inside the heel and are badly frayed. The newer shoes are scuffed and damaged a little bit.

I had a K2 mountain bike for 10 years and it had wide rectangular stays. On that bike I polished both sides but mostly the right. With some mtb shoes I have had to grind down the heel for more clearance.

Sometime when riding I'll hear a click or thump and it takes a second to remember it's my shoe hitting the bike.

A lady I know wore a deep groove, maybe 2mm, in the right crankarm.
