Actually...a team build of the Honda RN-01 Downhill bike had a CVT in it. Mountain Bike Action had a pretty good article covering how it works. It used a friction grip shifter.

Later RN-01s lacked the CVT, one rumor was the thing wsas outrageously expensive, so they had to cut costs, and moving back to a non-CVT drivetrain was the answer.
