JMHO:
1. Name (so if you're found unconscious there's somewhere to start)
2. Medical condition, if appropriate. Maybe. Blood thinner? Definitely. Diabetic? EMTs have a quick test strip.
3. Emergency contact + phone.
4. Passport? Are you riding in another country?
