I think I may have the same issue. I tried on some Sidi's and they were too narrow just before the toe area, and any shoe wide enough in the front was too large in the heel area. In all honesty the only pair of cycling shoes I've found that fit comfortably are from Specialized. Not quite as glamorous as Sidis but they do fit.
