Nowadays, 16mm is considered on the narrow side for 40mm tires, but it certainly won't cause any safety issues. This is not why you got the flat. Heck, some mountain bikes with 53mm tires used to come fitted with 17mm rims.

Hmmm. Is that the max pressure rating for the tire? The rims also have a max pressure rating which you may have exceeded. And if you are 300lbs, you definitely should not go to a narrower tire.

Also, what is the spoke count on your wheel? This is key for heavier riders. Get the highest spoke count possible.
