After further review, I don’t think my bike is a Miyata. Other differences identified on my bike:

Braze on water bottle holes missing on seat tube, braze on rear cable bracket on chain stay is different and cable bracket on seat tube appears to be lower then the position of the Miyata.

At first glance I thought you nailed it. I guess I will go back to this is a custom build from a no-name builder.
