Descente "classic pro" bibs will likely meet all these requirements (though I would qualify frontal coverage of the chamois as "average").

With respect to other brands/requirements: The chamois I've found that extends highest up the front is the Assis F.I Mille, and the fabric is very high quality and relatively thick, but the legs on those bibs are very short. The Castelli fabrics can be a little "lightweight," I've found, and the frontal chamois coverage can vary (very minimal on the YPRO-series, average on the Kiss) -- the Castelli fabrics are of high enough quality and seems to last fine, they just seem to be of lighter than average weight. You might want to try some of the Pearls as well, though the Ultrasensor and Microsensor fabrics would be what I consider sort of "midweight" fabrics. I have not ridden Giordana bibs in several years, but in all three of your requirements, I would consider them average, though depending on the model, the fabric may be lighter than most.
