Dia-Compe DC138 hand rests were typically paired with a set of Dia-Compe guidonnet-style brake levers for a more upright position on drop bars. I built up a cockpit in this style a decade ago but found guidonnet levers to be too flexy for my tastes:


Dia-Compe also makes the HR-07 "tandem hand rests":
