Things weren't much more evolved in the Lemond era. The first edition of The Custom Bicycle, published in 1979, included bike-fitting advice from a coach who had trained many national team members. His advice/rule regarding cleat positioning: set them so that your toes point in, for better aerodynamics.
That advice had disappeared by the time the next edition came out.
