Before moving on, you need to think about the underlying cause (or causes).

On a hunch. I suspect it's somehow related to the QR adaptor. Whether it's a loose adjustment, or that hub's design intolerance of axle compression, misaligned dropouts bowing the axle, or whatever----- Something started the process that led to failed internals.
