Bearings fail. An in depth analysis doesn't do much for you now. The point is, you should really replace the axle and the free hub body both. If you can't replace both of them together you need to replace the hub completely. That axle is compromised due to the bearing failure that occurred in the free hub. At the same time you can check the frame alignment and make sure it is good.
