I removed one that had cocked during installation on a thin section aluminum sailboat mast (which had originally been threaded which was why it stripped). So yes, I am aware that this requires caution and judgment, which I thought I said.

It may be possible instead, to grab the outer flange with pliers, and drill or dremmel-grind the rivnut until the inner bends of the back flange are gone and it pulls free, though this may leave part of the flange in the frame rattling around if cannot be removed at the BB.

But first, I would try reseating the original rivnut with a proper rivnut tool that does not require the splines to keep the rivnut from rotating. That is best.
