I have found it best to pass with the widest practicable berth or minimum speed disparity. Not really any different from the way we'd like cars to pass us.
If I'm approaching another rider or pedestrian who has shown no sign of acknowledging my hails, I'll give them as wide a berth as practical under the circumstances, for this exact reason - the possibility of an unpredictable action.
