I suffered from hot spots under the small toe at the joint with the metatarsal. Tried a wider cleat on a SPD-L pedal, from SPD. Didn’t solve it. Went to a shoe with a carbon mid-sole, Specialized mt, bike shoe and a stiff sole solved the problem. Haven’t had hot spot pain in years.
