The 'loose' cap is L hand threaded and only installs on the L/non-drive side. The threads on the cartridge are R handed
threads and mount from the drive side and should screw in easily for several turns before requiring a tool to fully tighten.
Bbkt drive side is left hand thread, NDS is right hand thread (pedals are opposite - NDS pedal has left hand thread).
