It appears that the cartridges in the pics are from a different caliper, possibly from BR-6403. Not sure if that is hurting your reach but the bolt could be the older 6mm style and the cartridge/pad could be 'thicker'.
Another factor could be the outer width of the rim. Common rims at the time of the 6500 Ultegra were quite narrow.
