Re. noisy pads, course file the braking surface, really get after it, works every time for me, has to be very course mill bastard, the bigger the better IMO.

Pad holders are a pita to wrangle when you have to try and force them open or get the pads out, especially when the holders are closed on all 4 sides.

I bolt them to a metal plate that can be clamped down to the bench so I can force them into what I want, I can normally pry one end open so I can pry/force the pad out from the other end or get underneath it and just force it up and out, dishsoap can work for lube to get the tool or screwdriver in and help the pad out.
