She needs more torque because she moved from a flat to a hilly area. As an alternative to changing the primary gear ratio, we were wondering if installing a beefy rear hub might solve the problem.

I did mean the Sturmey-Archer's CS-RK3 combined with a 7-9s cassette.
But it sounds like she could use more in depth help as you have been going after odd solutions which will make it harder for her. She might not know what is out there and might need some guidance on that.

You didn't mention that hub you said 3 speed which leads me to believe it was an after thought but I don't think that hub would be a huge improvement that going to the gear ratio I had suggested earlier would be the best way to go. The hub she has is a great hub designed for bikes like that but in the end it is a super heavy 3 wheeled trike designed around flatter terrain so there is not a great solution for it beyond removing weight which is a tough thing as you need durability.
