Thank you all for the suggestions on fit and bars, but I didn't want to go into my background as I would have made the OP too long.
Based on the number of fit and aero recommendations here is more detail on why the decision to do it is already made. I just want to do it buying the least amount of equipment.

I've determined I will do this to see if it improves things, so I was really looking for input on those that have done it. Thanks to Darth Lefty for the pictures.

This is mostly the result of a neck disc/nerve pinch (from a 40+ year ago face plant) and then age, fitness, weight more than than fit or anything else. This is a physical thing, not a standard fit thing. Things are not bad enough for surgery, but I have had medical procedures done, MRIs etc. I am looking into neck decompression. I know fit changes depending on what you are doing and the other things I listed.
If I sit at my desk with my arms in front I will get a burning pain just like I do when riding on the hoods. I have many bikes and have had many others. Things have changed over the years such that since I
Example - I can't row a Kayak without pain, but I can row a row boat, paddle a canoe, do SUP.

So anyway...More input on if you did it, what you changed and how it worked is appreciated.
