The original plastic top cap is useless and will not withstand the required torque. However by the looks of the pictures you may have overtorqued your set up. There is no problem with the 2 piece spacers. Get some new spacers and buy an aluminium cap from the ebay link I put earlier in the thread, then be sure to use a torque wrench. No need to go higher than 3.5Nm initially. Then back it off and re-tighten to about 2/2.5Nm. At 3.5Nm you should be able to feel some resistance in the steering when bike is on a stand. At 2.5N/m that resistance should be negligible. Check for play after 1st decent ride and retorque as required.
