I'm a fan of quill adapters that let me mix the stems and bars as required to get the fit I want. You can find 22.2 quills with tops to fit 25.4 or 28.6 threadless stems and those stems to fit various bar clamps. I've had a couple of the 28.6 tops turned down when necessary.
