If the extra tab is on the outside of the shoe how the hell is it getting caught
on the crank arm (inside of shoe?).

There is another tab that sticks out when sizing the soft arch compression strap (opposite the buckle) on the inside of the shoe (SIDI) that can be easily caught on the crank arm if it is sticking out too far. SIDI's own instructions state to trim the excess after sizing.
I am ASSUMING this is what we're talking about...I know...never assume....
