+1 to GrannyGear's remarks. I always ask, because of 2 reasons. 1) I'd want someone to offer help to me; 2) I really do believe that every interaction we have with another can help change the universe.

+1 to Big Paulie's response. This has been my experience. Most times they don't need the help. Occassionally, they don't want the help, but sometimes they need the help and you can be the one to provide it.
In the last year I must have asked others if they needed help at least a ten times, and the offer was taken on two of those times. I rode away from both enjoying the remainder of my ride just a bit more.
