My old water bottle cage once ejected the bottle, and the nylon cage end-piece (cage was metal, otherwise) right into a turn lane as I hit a bump. Of course, I noticed it happen as I had to dodge hitting it with my rear wheel as I passed it, then had to safely stop, get off the bike and grab it before my Brita-Filter bottle got smushed by traffic.
Since then I've upgraded to a cage made with thicker metal and no cheap nylon pieces.
