It's happened to me twice now... It's only a matter of time.
And that's WITH a front brake. Sometimes you just can't get to it quickly enough if your hands aren't already on it. But yeah, that's another good reason for taking the lane: you only have to swerve halfway around a car instead of the full-width if you're riding down the center of the lane.
