Tire Levers
CO2 pump

Cellphone (with In Case of Emergency ICE numbers for anyone who finds me battered and unconscious)
Laminated copy of drivers license and insurance card (both highly recommended for people repair!!!)

When riding on roads with traffic, fixing yourself is far more critical than fixing your bike, and often overlooked.
