Shoe hotspots

I have Time ATAC pedals, and Nashbar "pro" mountain shoes..

The shoes seem to be a fine fit, but after about 5 miles I begin to develop a hotspot on my left foot... basically where the shoe meets the pedal.

The cleats are in the same position for both the left and right shoes, but as I pedal it doesn't feel like they are positioned in the same area on my feet. I've tried moving the left cleat back a few mm, but that didn't seem to do much.

In general it seems one shoe fits a little better than the other... are my feet different sizes?

Any ideas as to how I can get rid of it? It's getting really annoying!

Thanks!
