Doesn't really appear that the cog slipped. I have the Miche carrier and it was drenched in locktight prior to assembly. Plus it has a bunch of road grime all along the crevice between the lockring and the carrier that would have showed signs of slippage.

I actually also hit the ground on my right pocket (where I keep my keys which were smashed into my leg) and I also had a lady ask me if I was ok seconds post crash (not a cop); and while I was rocking my bike a bit, I don't think cones is the issue. I have the newer Miche hubs with the sealed bearings, that couldn't happen with sealed bearings....could it? And no I didn't catch my seat on my ass.
