It seems like vans and other shoes are thick soled enough to make it work.
most aren't really. I've seen it done and the cleat usually hits the ground a lot more then with real cycling shoes. Thats why all the skate shoe style cycling shoes look so ridiculous.
If you do want to do it.
-rip out the insole and cut out the cleat area.
-get some thick plastic and cut it in the shape of the insole
-dremel slots for the cleat.
-put everything in place then get a new insole to cover cleat.
