It definitely does not require hard cornering, though that would make it more likely I suppose.
Certainly when it does happen, it's going to be some weakest link that gives way - such as one point on the bead seat, but that is not to say a defective rim.
The hotter you get the rims, the more likely it is to happen.
The higher pressure you start with, the more likely it is to happen.
I suspect wider tires are more likely than narrow ones to blow out.
I suspect older, thinner rims are more likely allow a blow out.
Mostly though, it happens because the rider doesn't understand that it can happen and doesn't know how to prevent it from happening. It happened to me
ONCE! and now I'm always conscious of it and won't let it happen again. Most folks have a similar story.
