Shimano now makes freewheels that have a 10-tooth jump between the last two cogs, from 24 to 34. The 34t cog is conceptualized as a "bail-out" gear. Seems to work fine, some friends of mine have this setup on their bikes (cheap-ish mtb-gearing bikes ridden around town). Of course, these have HG-ramped teeth.
