Perhaps a combo? Put a line to mark the edge of the parking zone, and put in a sharrow centered where the left hand bike lane line would be if the bike lane were 6 feet wide. Encourage cyclists to ride on top of the sharrow, and they will be out of the door zone and out of faster traffic, or at least out of it enough that faster traffic can pass without much friction. As part of the sharrow symbol, perhaps a "door zone" and a "do not bike here" message can be added to warn cyclists away from the door zone.
I agree except that I think the sharrows marking should be slightly biased left of center rather than right of center in the lane, at least if the lane is one of those 'too narrow to share' lanes. OTOH, six feet out in a sub-12 foot lane probably would be slightly biased left of center.
