I don't think its an injury issue..

I don't use KOPS, but my knee position is about a centimeter back when the pedal is at 3 o'clock. I used to have it dead on, now I have noticed a slight increase in power in my pedal strokes.

The only problem with using a too big or too little frame is you tend to adjust yourself to the bike. The bike is suppose to adjust to the rider, not the other way around. If you get the right sized frame, you can adjust the saddle poistion, tilt as well as stem length until it feels really good.
