I routed the two cables parallel with each other and close together on the inside of the bars. That means you have to route the shift cable housing on the inside channel of the brifter body so it stays close to the brake cable housing. It seems to produce a rather sharp bend in the housing but the bike shifts fine.

My bars are FSA and only have one cable groove but the cable routing is good and the bars aren't too fat feeling.
