I broke both sides of my IRO rear, with no adjustments, after three years of use. I replaced them with super generic steel nuts with the same threading (got them from a bike co-op in Montreal) and have never had a problem since. I wouldn't bother with replacements from IRO, just find something local. Shouldn't be too hard in most cities.
