unless you can tighten each bolt at the same time with the exact amount of pressure, it is unavoidable to put more pressure on one contact point (a small part of the steerer tube) than the other. This gets worse the closer you get to the max torque. I snapped a 1/2" off the top of a Colnago Star tube with that stem. Thankfully I was able to go 1/2" more aero.......
